What it is

hCG is the hormone that pregnancy tests detect. The placenta makes it to keep the ovary producing progesterone in early pregnancy. It is a large two-part glycoprotein, not a small peptide, and it acts almost exactly like LH: in men it tells the Leydig cells in the testes to make testosterone, and in women it can trigger the final step before ovulation.

In medicine hCG is an old, approved drug. Pregnyl and Novarel are purified from pregnancy urine and injected into muscle; Ovidrel is a recombinant version injected under the skin. Labeled uses are undescended testes in boys, low testosterone caused by pituitary failure in men, and triggering ovulation in fertility treatment. Men's clinics also give low doses alongside testosterone therapy to keep the testes making testosterone and sperm. That is off-label, and the evidence is a 29-man hormone study and a 26-man chart review, not a randomized fertility trial.

hCG is also the center of the 'hCG diet', a 500-calorie-a-day plan from the 1950s. Controlled trials found people lost the same weight on placebo injections, and the Novarel and Pregnyl labels state hCG has no known effect on fat, appetite or hunger. Since March 23, 2020, FDA has regulated hCG as a biologic, which ended compounding pharmacy production. WADA bans it in male athletes.

Mechanism

How hCG (Human Chorionic Gonadotropin) works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target LH/choriogonadotropin receptor (LHCGR), agonist

hCG binds the same receptor as LH, the LH/choriogonadotropin receptor (LHCGR), on Leydig cells in the testes and on ovarian follicle and corpus luteum cells. In men that drives testosterone production inside the testes, which sperm production depends on. In women a large dose mimics the mid-cycle LH surge, finishing egg maturation and triggering ovulation about 36 hours later. Because it bypasses the brain and pituitary, it works even when the pituitary has been shut down by testosterone therapy.

  1. 1 Injection Urinary hCG is injected into muscle; recombinant hCG (Ovidrel) under the skin, reaching peak levels in 12 to 24 hours.
  2. 2 Receptor binding hCG binds LHCGR on testicular Leydig cells or on ovarian theca, granulosa and corpus luteum cells.
  3. 3 Steroid production Leydig cells make testosterone; in one study testicular testosterone was about 80 times higher than blood testosterone.
  4. 4 Downstream effect Men: testicular testosterone supports sperm production and testicular size. Women: final egg maturation and ovulation, then progesterone from the corpus luteum.
  5. 5 Clearance Terminal half-life is about 29 hours, so effects of one dose last days.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

hCG is an established fertility and hypogonadism drug backed by decades of use and many randomized trials in IVF. For men with pituitary-caused hypogonadism, gonadotropin therapy produces sperm in about 75% of cases. Its off-label use to preserve testicular function on testosterone therapy rests on small studies, and controlled trials show it does nothing for weight loss.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Whether hCG on testosterone therapy preserves fertility in a randomized trial; current evidence is a 3-week hormone study and a 26-man chart review.
  2. 02 The best dose to restart natural testosterone after anabolic steroid use; no controlled trials exist.
  3. 03 Long-term safety of continuous hCG use for years in men on TRT.
  4. 04 How estradiol and red blood cell levels change with long-term hCG plus testosterone.
  5. 05 Potency and contents of hCG sold online or as 'research' product outside licensed pharmacies.

Pharmacokinetics

How long it stays in the body

Half-life
About 29 hours
About 29 hours terminal (recombinant hCG, subcutaneous); initial phase about 4.5 hours
Routes studied
Intramuscular injection (urinary hCG: Pregnyl, Novarel), Subcutaneous injection (recombinant hCG: Ovidrel)
Bioavailability
About 40% after subcutaneous injection of recombinant hCG; peak levels at 12 to 24 hours
Notes
About one-tenth of a dose is excreted in urine, which is why injected hCG makes pregnancy tests positive for days. hCG also cross-reacts in some LH blood tests. Not active by mouth, sublingually or as 'homeopathic' drops.

Interactions
  • Menotropins and FSH in fertility treatment: the combination drives ovarian hyperstimulation risk and multiple pregnancy.
  • LH and pregnancy blood and urine tests: hCG cross-reacts, giving false LH readings and positive pregnancy tests for days after a dose.
  • Testosterone therapy: hCG is used with it off-label; the combination raises estradiol and testosterone further than either alone.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"hCG injections plus a 500-calorie diet burn fat without hunger."Not supportedThe evidence does not back this

Double-blind trials found the same weight loss on placebo injections, a 1995 meta-analysis found no benefit, and the Novarel and Pregnyl labels say hCG has no known effect on fat or hunger. The weight loss comes from eating 500 calories a day.

The claim"Over-the-counter hCG drops work like the injections."Not supportedThe evidence does not back this

FDA says all 'homeopathic' hCG products are illegally marketed. hCG is only approved as a prescription injection, and it is not absorbed under the tongue.

The claim"hCG keeps your testicles working while on testosterone."SupportedHuman evidence backs this

In 29 men on testosterone, 250 to 500 IU every other day kept testicular testosterone near normal, and in a 26-man series no one became azoospermic. These are small studies, not fertility RCTs.

The claim"hCG fully restarts natural testosterone after a steroid cycle."UntestedNobody has measured this in people

It is widely used for this, but no controlled trial has measured recovery of the body's own testosterone with versus without hCG.

The claim"hCG is a peptide you can get from any compounding pharmacy."Not supportedThe evidence does not back this

Since March 23, 2020, hCG has been a licensed biologic and US compounding pharmacies can no longer make it.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

hCG with testosterone therapy

Testosterone suppresses LH and FSH; hCG replaces the LH signal so the testes keep making testosterone and, likely, sperm.

EvidenceSupported by a 29-man, 3-week hormone study and a 26-man retrospective series. No randomized trial with pregnancy as the outcome.

Handling

Storage and stability

Frequently asked questions
Does the hCG diet work?
The diet causes weight loss because it limits you to about 500 calories a day. The hCG adds nothing: in controlled trials people lost the same weight on placebo shots, and the Novarel and Pregnyl labels state hCG has no known effect on fat or hunger.
Why do men on TRT take hCG?
Testosterone shuts off LH, so the testes shrink and sperm production falls. hCG acts like LH and keeps the testes making testosterone. Small studies support it; it is off-label.
Can I still get compounded hCG?
No. Since March 23, 2020, hCG is regulated as a biologic and compounding pharmacies cannot make it. Licensed products include Pregnyl, Novarel, generic chorionic gonadotropin and Ovidrel.
Will hCG make a pregnancy test positive?
Yes. Pregnancy tests detect hCG, so an injection can produce a positive test for days afterward. It also cross-reacts with some LH blood tests.
Is hCG banned in sports?
For men, yes. WADA lists chorionic gonadotrophin as a prohibited testosterone-stimulating peptide in males. It is not prohibited for women.
What is the difference between Pregnyl, Novarel and Ovidrel?
Pregnyl and Novarel are hCG purified from pregnancy urine and injected into muscle. Ovidrel is recombinant hCG made in cell culture and injected under the skin, approved for fertility treatment.
