What it is

Semaglutide is a modified copy of GLP-1, a hormone your gut releases after a meal. The natural hormone lasts about two minutes in the blood. Semaglutide has a fatty acid tail that sticks to albumin, so one injection lasts about a week. It is sold by Novo Nordisk as Ozempic (type 2 diabetes), Wegovy (weight, heart risk and fatty liver) and as daily tablets (Rybelsus, Ozempic tablets and Wegovy tablets).

The weight data are large and consistent. In STEP 1, 1,961 adults without diabetes lost an average of 14.9% of body weight over 68 weeks on 2.4 mg weekly, against 2.4% on placebo. The higher 7.2 mg dose (Wegovy HD, approved March 2026) averaged 18.7% in STEP UP. Outcome trials go past the scale: SELECT cut heart attack, stroke and cardiovascular death by 20% in 17,604 people with heart disease and excess weight, and FLOW cut serious kidney events by 24% in people with type 2 diabetes and kidney disease.

The main costs are nausea, vomiting, diarrhea and constipation, mostly during dose increases, plus gallbladder disease and rare pancreatitis. Weight comes back when the drug stops: in the STEP 1 extension, people regained about two thirds of the lost weight within a year. Semaglutide is an approved prescription drug, so the questions worth asking are about fit, side effects and how long to stay on it, not whether it works.

Origin
Synthetic analog of human GLP-1 (7-37). Position 8 is swapped to aminoisobutyric acid (Aib) to block DPP-4 breakdown, position 34 lysine is swapped to arginine, and lysine 26 carries a C18 fatty diacid on a spacer that binds albumin. The peptide backbone is made by yeast fermentation.
Mechanism

How Semaglutide works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target GLP-1 receptor (full agonist)

Semaglutide binds and activates the GLP-1 receptor on pancreatic beta cells, in the stomach and in appetite centers of the brainstem and hypothalamus. It raises insulin and lowers glucagon only when blood sugar is elevated, which is why it rarely causes low blood sugar on its own. It slows stomach emptying and lowers appetite and food cravings, so people eat less without trying to.

  1. 1 Albumin binding After injection the C18 fatty acid tail binds albumin in the blood, which shields the drug from kidney filtration and enzyme breakdown and stretches the half-life to about one week.
  2. 2 Receptor activation Semaglutide activates GLP-1 receptors on beta cells, raising insulin release and lowering glucagon in a glucose-dependent way.
  3. 3 Slower stomach emptying Food leaves the stomach more slowly, so people feel full sooner and post-meal sugar spikes are smaller. This effect is strongest in the first weeks.
  4. 4 Brain appetite signals The drug reaches the area postrema and hypothalamus, where it lowers hunger and the urge to eat high-fat food.
  5. 5 Downstream effects Lower calorie intake drives weight loss, and weight loss plus direct vascular and anti-inflammatory effects are thought to explain the drop in heart and kidney events seen in SELECT and FLOW.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

This is one of the best-studied drugs in medicine. Large placebo-controlled trials show average weight loss of about 15% at 2.4 mg and about 19% at 7.2 mg, fewer heart attacks and strokes (SELECT), slower kidney decline (FLOW), better heart failure symptoms (STEP-HFpEF) and resolution of fatty liver inflammation (ESSENCE). What is not shown in humans: protection against Alzheimer's disease (the EVOKE trials were negative in 2025) or any effect on aging itself.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Whether staying on semaglutide for decades is safe: the longest controlled data run about 4 to 5 years.
  2. 02 Whether rodent thyroid C-cell tumors have any human relevance; no clear human signal so far, but rare cancers need very long follow-up.
  3. 03 How much of the lean mass lost is skeletal muscle versus organ and water mass, and whether it affects strength or falls in older adults.
  4. 04 The best way to stop or taper without regaining weight; the STEP 1 extension showed about two thirds of lost weight returned within a year.
  5. 05 Effects in pregnancy and breastfeeding, which have not been studied in trials.
  6. 06 Whether compounded or salt-form semaglutide matches the approved product in potency, purity and safety.

Pharmacokinetics

How long it stays in the body

Half-life
About 7 days
Routes studied
Subcutaneous injection (once weekly), Oral tablet (once daily)
Bioavailability
89% after subcutaneous injection; about 1% to 2% for oral tablets, which use the absorption enhancer SNAC and must be taken on an empty stomach.
Notes
Peak levels arrive 1 to 3 days after an injection. Because of the one-week half-life the drug stays in the blood for about 5 to 7 weeks after the last dose, which is why the label advises stopping at least 2 months before a planned pregnancy.

Groups with specific risks
  • Personal or family history of medullary thyroid carcinoma
  • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
  • Prior serious hypersensitivity reaction to semaglutide or product ingredients
  • Pregnancy is not a listed contraindication, but the label says it may cause fetal harm: stop when pregnancy is recognized and at least 2 months before a planned pregnancy
Interactions
  • Insulin or sulfonylureas: higher risk of low blood sugar; doses of those drugs are often lowered.
  • Oral drugs with a narrow therapeutic range (for example warfarin, levothyroxine): slower stomach emptying can change absorption, so levels may need checking.
  • Other GLP-1 or tirzepatide products: not to be combined.
  • General anesthesia or deep sedation: reports of pulmonary aspiration because food stays in the stomach longer; tell the surgical team you take it.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Semaglutide causes thyroid cancer in people."MixedSome studies agree, some do not

It causes thyroid C-cell tumors in rats and mice, which is why the label carries a boxed warning and excludes people with MTC or MEN 2. Large human trials and registries have not shown a clear increase, but rare cancers take years to detect, so the question is not closed.

The claim"You will regain all the weight when you stop."MixedSome studies agree, some do not

Most people regain a large share. In the STEP 1 extension, people regained about two thirds of their lost weight within a year of stopping and most heart risk markers drifted back toward baseline. Some people kept part of the loss, but stopping without a plan usually means regain.

The claim"Most of the weight lost is muscle."Not supportedThe evidence does not back this

Lean mass made up roughly 40% of the weight lost in the STEP 1 body-scan subgroup, so most of the loss was fat. That share sits at the high end of what is reported for diet-only weight loss. Lean mass includes water and organs, not just muscle. Resistance training and enough protein are the standard ways to limit muscle loss.

The claim"Compounded semaglutide is the same drug as Wegovy."Not supportedThe evidence does not back this

Compounded products are not FDA-reviewed for identity, strength or purity, and some used semaglutide sodium or acetate salts, which FDA says are different active ingredients. FDA has logged dosing errors and adverse events tied to compounded versions.

The claim"Semaglutide raises the risk of suicide."Not supportedThe evidence does not back this

FDA and EMA reviews of trial and real-world data did not find a causal link, and in early 2026 FDA removed the suicidal behavior warning from the Wegovy label. Anyone with mood changes on any drug should still tell their clinician.

The claim"Semaglutide prevents Alzheimer's disease."Not supportedThe evidence does not back this

The phase 3 EVOKE and EVOKE+ trials of oral semaglutide in 3,808 people with early Alzheimer's disease found no slowing of decline versus placebo over 104 weeks (results announced in 2025, published in the Lancet in 2026).

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

CagriSema (cagrilintide + semaglutide)

Amylin and GLP-1 reduce appetite through different brain circuits, so adding the amylin analog cagrilintide was designed to add weight loss beyond semaglutide alone.

EvidenceStudied in phase 3. In REDEFINE 1, 3,417 adults lost 20.4% on the combination vs 3.0% on placebo at 68 weeks. In REDEFINE 4 it did not match tirzepatide 15 mg. Filed with FDA in December 2025; not approved as of October 2026.

Semaglutide + tirzepatide or another GLP-1

Sometimes discussed online as a way to push weight loss further.

EvidenceNever studied together. Both labels say not to combine products that act on the GLP-1 receptor; overlapping drugs raise GI and dehydration risk with no shown benefit.

Frequently asked questions
What is the difference between Ozempic and Wegovy?
Same molecule, different labels and doses. Ozempic is approved for type 2 diabetes (up to 2 mg weekly) and its heart and kidney complications. Wegovy is approved for weight management, heart risk reduction and MASH, at a 2.4 mg maintenance dose, with a 7.2 mg option (Wegovy HD) and a 25 mg daily tablet.
How much weight do people lose on semaglutide?
In STEP 1 the average was 14.9% of body weight over 68 weeks at 2.4 mg, against 2.4% on placebo. Half of participants lost at least 15%. At 7.2 mg the average was 18.7% over 72 weeks. Individual results vary widely.
What happens when you stop taking semaglutide?
Appetite returns within weeks as the drug clears. In the STEP 1 extension, people regained about two thirds of the lost weight within a year, and blood pressure and blood sugar moved back toward where they started.
Is semaglutide safe long term?
Trials up to about 4 years, including SELECT with 17,604 people, show fewer heart events and no new major safety signal beyond the known GI effects, gallbladder disease, rare pancreatitis and very rare NAION. Data beyond 5 years are limited.
Is semaglutide banned in sports?
No. It is not on the WADA Prohibited List. WADA monitors it, and from 2026 also tirzepatide, to look for patterns of misuse.
Is compounded semaglutide still legal?
The shortage ended February 21, 2025. After the grace periods closed (April 22, 2025 for pharmacies, May 22, 2025 for outsourcing facilities), compounding copies of the approved product is no longer permitted except in narrow patient-specific cases.
