What it is

Retatrutide is a 39-amino-acid peptide that activates three hormone receptors at once: GIP, GLP-1 and glucagon. The first two cut appetite and improve blood sugar, the same way tirzepatide does. The glucagon arm is the new part: glucagon tells the liver to burn fat and raises energy use, which is why the liver fat results stand out. One injection lasts about 6 days.

The phase 3 TRIUMPH program finished its core trials in 2026. In TRIUMPH-1, 2,339 adults with obesity and no diabetes lost an average of 25.0% of body weight over 80 weeks on 12 mg, against 3.9% on placebo (counting everyone randomized; Lilly's efficacy-estimand figure, which estimates the result if everyone had stayed on treatment, is 28.3%). In people with type 2 diabetes (TRIUMPH-2), the average was 18.8%. Knee arthritis pain and sleep apnea events also fell. In a 2024 substudy, 86% of people with fatty liver on 12 mg reached normal liver fat by 24 weeks.

Retatrutide is not approved anywhere. Lilly has said it plans to file with the FDA in the first quarter of 2027. The side effects so far are the usual GI ones plus two that need watching: a dose-related rise in heart rate and dysesthesia (burning or tingling skin) in up to 21% at 12 mg. Vials sold online as research chemicals are unapproved drugs of unknown content, and FDA has sent warning letters to sellers since 2024.

Origin
Synthetic peptide built on a GIP-like backbone with non-natural amino acids for stability and a C20 fatty diacid side chain that binds albumin. Engineered by Eli Lilly to activate GIP, GLP-1 and glucagon receptors from one molecule. Exact published sequence and molecular weight were not verified for this record.
Mechanism

How Retatrutide works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target GIP receptor, GLP-1 receptor and glucagon receptor (triple agonist)

Retatrutide is most potent at the GIP receptor and somewhat less potent than the native hormones at the GLP-1 and glucagon receptors. GIP and GLP-1 activity lower appetite, raise glucose-dependent insulin and slow stomach emptying. Glucagon activity raises energy expenditure and fat burning in the liver, which may explain why weight and liver fat fall further than with dual agonists. The glucagon effect would raise blood sugar on its own, but the GLP-1 and GIP arms more than offset it.

  1. 1 Albumin binding The fatty acid side chain binds albumin, giving a half-life of about 6 days and once-weekly dosing.
  2. 2 GIP and GLP-1 activation Activation of these two incretin receptors lowers appetite through the brain and raises insulin only when glucose is high.
  3. 3 Glucagon receptor activation Activation of the glucagon receptor in the liver increases fat oxidation and energy expenditure, and drives down liver fat.
  4. 4 Balanced glucose effect Glucagon raises blood sugar, but the incretin arms outweigh it, so HbA1c still falls about 1.5 to 2 points in type 2 diabetes.
  5. 5 Net result Lower food intake plus higher energy use produce average weight loss of about 25% at 80 weeks in people without diabetes.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Multiple randomized trials, including at least four phase 3 trials with about 6,000 participants combined, show large weight loss (about 19% to 25% at 80 weeks on the treatment-regimen analysis), lower HbA1c in type 2 diabetes, less knee pain and fewer sleep apnea events. What is missing: approval, any trial longer than about 2 years, and outcome data on heart attacks, strokes or death. The cardiovascular signal in TRIUMPH-3 was too small to read either way.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Safety beyond about 2 years of treatment.
  2. 02 Whether retatrutide lowers heart attacks, strokes or death; TRIUMPH-3 was not powered to answer this and its numbers were inconclusive.
  3. 03 What causes the dysesthesia signal and whether it resolves in everyone.
  4. 04 Long-term effect of glucagon receptor activation on heart rate, bone and lean mass.
  5. 05 How much of the weight lost is skeletal muscle.
  6. 06 Weight regain after stopping; no published withdrawal trial yet.
  7. 07 Content, purity and sterility of research-chemical vials sold online, which have never been tested against the trial drug.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Retatrutide is FDA approved or about to be sold in pharmacies."Not supportedThe evidence does not back this

It is not approved anywhere as of October 2026. Lilly has said it plans to file in the first quarter of 2027, and a standard FDA review then takes about 10 to 12 months.

The claim"People lose 30% of their body weight on retatrutide."MixedSome studies agree, some do not

Lilly's headline numbers (28.3% in TRIUMPH-1, and 30.3% at 104 weeks in a blinded extension limited to people with BMI 35 or higher) use the efficacy estimand at the 12 mg dose, which estimates the result if everyone had stayed on treatment. Counting everyone randomized, the 80-week average was 25.0% on 12 mg in people without diabetes and 18.8% in people with type 2 diabetes.

The claim"Research-grade retatrutide from peptide websites is the same as the trial drug."UntestedNobody has measured this in people

No independent testing ties those vials to Lilly's molecule, dose or sterility. FDA calls them unapproved new drugs, and FDA adverse event reports have recorded hospitalizations linked to unapproved retatrutide.

The claim"Retatrutide preserves muscle because glucagon burns fat."UntestedNobody has measured this in people

Detailed body composition data from phase 3 have not been fully published. There is no human evidence yet that retatrutide spares muscle better than tirzepatide or semaglutide.

The claim"Retatrutide has no new side effects compared with Ozempic."Not supportedThe evidence does not back this

Besides the usual GI effects, it causes dysesthesia (burning or tingling skin) in up to about 1 in 5 people on 12 mg in one trial, a dose-related heart rate rise, and more low blood pressure than placebo.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Retatrutide + cagrilintide

Discussed online as a way to add amylin-driven fullness on top of the triple agonist, by analogy with CagriSema.

EvidenceAnimal data only: a 2026 study in obese male rats reported more weight loss with the combination. No human trial has tested it.

Retatrutide + tesamorelin or GH secretagogues

Claimed online to protect muscle during rapid weight loss.

EvidenceNo human or animal study of these combinations exists. Rationale is speculative.

Frequently asked questions
Is retatrutide FDA approved?
No. It is in phase 3 trials. Lilly has said it plans to submit to the FDA in the first quarter of 2027. A standard FDA review takes about 10 to 12 months, so a decision before late 2027 would require a faster review.
How much weight do people lose on retatrutide?
In TRIUMPH-1, adults without diabetes averaged -25.0% at 80 weeks on 12 mg counting everyone randomized, and 28.3% on Lilly's efficacy estimand, which estimates the result if everyone had stayed on the drug. People with type 2 diabetes averaged -18.8%.
How is retatrutide different from tirzepatide?
Tirzepatide activates GIP and GLP-1 receptors. Retatrutide adds the glucagon receptor, which raises energy use and fat burning in the liver. In separate trials it produced more weight loss than tirzepatide, but no head-to-head trial has been published.
What are the side effects of retatrutide?
Mostly nausea, diarrhea, vomiting and constipation during dose increases. It also caused burning or tingling skin (dysesthesia), a dose-related rise in heart rate and more low blood pressure than placebo.
Is it legal to buy retatrutide online?
In the US it is not legally sold for human use. Products labeled research use only are unapproved drugs; FDA has sent warning letters to sellers and their content and sterility are unknown.
Is retatrutide banned in sport?
It is not named on the WADA list, but as a drug with no government approval for human use it falls under WADA's S0 non-approved substances category, which is prohibited at all times.
