What it is

Tesamorelin is the full 44-amino-acid human growth hormone-releasing hormone with a small fatty acid chain bolted onto the front end so blood enzymes cannot break it down as fast. It tells the pituitary to release more of its own growth hormone. Sold as Egrifta, it is the only drug FDA has approved to reduce excess abdominal fat in adults with HIV and lipodystrophy, a fat redistribution caused by older HIV medicines.

The evidence is strong for that one use. In two phase 3 trials with 806 people, daily 2 mg injections cut visceral fat, the fat packed around the organs, by about 15% over 26 weeks, while fat under the skin and body weight barely moved. Triglycerides fell. When people stopped, the visceral fat came back within months. Smaller trials found it lowered liver fat: in a 2019 trial of 61 people with HIV and fatty liver, liver fat fell 37% relative to baseline. A 2012 trial in 152 older adults found better scores on executive function tests after 20 weeks.

The tradeoffs are real. Nearly half of patients developed antibodies to the drug, 47% had IGF-1 above the normal range after 26 weeks (more than 2 SDS, with 36% above 3 SDS), joint pain and swelling were common, and the risk of developing diabetes was about 3 times higher than on placebo. The label states it is not a weight-loss drug. The EU never approved it: the sponsor withdrew its application in 2012 after regulators signaled the benefit-risk balance was not shown.

Mechanism

How Tesamorelin works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target GHRH receptor on anterior pituitary somatotrophs (agonist)

Tesamorelin binds GHRH receptors on the pituitary and increases the pulsatile release of growth hormone, which raises IGF-1 and IGFBP-3. Growth hormone breaks down stored fat, and visceral fat responds more than fat under the skin, which is why waist size and organ fat fall while weight changes little. The hexenoyl group protects the front of the molecule from enzyme attack, but the drug is still cleared from blood within minutes; its effect lasts because the GH and IGF-1 changes last.

  1. 1 Injection under the skin Given as a once-daily injection into the abdomen. Peak blood level comes about 9 minutes later.
  2. 2 Pituitary signal Binds GHRH receptors and increases the natural pulses of GH release; feedback from somatostatin still applies.
  3. 3 IGF-1 rises In the first phase 3 trial, IGF-1 rose 81% on tesamorelin and fell 5% on placebo.
  4. 4 Visceral fat breakdown GH drives lipolysis, most strongly in visceral fat: about 15% loss over 26 weeks, with subcutaneous fat unchanged.
  5. 5 Effect reverses Stopping the drug lets visceral fat return; continued use kept it down at 52 weeks.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Two phase 3 RCTs (806 patients) and a 2026 meta-analysis of five RCTs show daily tesamorelin reduces visceral fat by roughly 15-18% and liver fat by about 4 percentage points in people with HIV, without lowering body weight or subcutaneous fat. Evidence outside HIV is limited to one RCT in older adults (better executive function, 7.4% less body fat) and small studies; it has never been tested as a general obesity drug.

Groups with specific risks
  • Disrupted hypothalamic-pituitary axis from pituitary removal, hypopituitarism, pituitary tumor or surgery, head radiation or head trauma
  • Active cancer, or prior cancer that is not inactive with treatment complete
  • Known hypersensitivity to tesamorelin or the product's excipients
  • Pregnancy
  • Breastfeeding in people with HIV
  • Acute critical illness after open heart or abdominal surgery, multiple trauma or acute respiratory failure: a label warning rather than a formal contraindication, because GH raised mortality in these settings; the label advises considering stopping it
Interactions
  • Drugs cleared by cytochrome P450 enzymes (corticosteroids, sex steroids, anticonvulsants, cyclosporine): GH may change their clearance, so the label advises monitoring; simvastatin (CYP3A) was not significantly affected in a study.
  • Insulin and other glucose-lowering drugs: tesamorelin can raise glucose and HbA1c.
  • Glucocorticoid replacement: GH inhibits 11-beta-HSD-1, which converts cortisone to cortisol, so people on replacement doses for adrenal insufficiency may need dose increases (label).
  • Other GH-axis agents (CJC-1295, ipamorelin, MK-677, growth hormone) would stack IGF-1 exposure and have not been studied together.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Tesamorelin is a weight-loss drug."Not supportedThe evidence does not back this

The label says it is not indicated for weight loss. In trials it cut visceral fat about 15% while body weight, BMI and fat under the skin barely changed.

The claim"Tesamorelin specifically targets belly fat."SupportedHuman evidence backs this

In people with HIV lipodystrophy it reduced visceral fat, trunk fat and waist size without changing subcutaneous belly fat. That is the basis of its approval.

The claim"The fat stays off after you stop."Not supportedThe evidence does not back this

In the extension phase, people switched to placebo regained the visceral fat they had lost; those who stayed on drug kept it off.

The claim"Tesamorelin improves memory and brain function."MixedSome studies agree, some do not

A 152-person trial in older adults found better executive function after 20 weeks. A 2025 trial in people with HIV found no difference in cognition versus standard care.

The claim"It works the same in people without HIV."UntestedNobody has measured this in people

Almost all trials enrolled people with HIV. The older-adult trial found 7.4% less body fat, but there is no obesity trial in the general population.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Tesamorelin + GLP-1 agonist

Pairs a drug that shrinks visceral fat while sparing lean mass with a drug that cuts overall weight through appetite.

EvidenceNo trial has combined them. Both can affect glucose regulation in different directions, and no safety data exist for the pair.

Tesamorelin + ipamorelin

Adds a ghrelin-receptor trigger to the GHRH signal to raise GH further.

EvidenceNever studied. Ipamorelin is unapproved, and stacking raises IGF-1, which already exceeds 3 SDS in about a third of patients on tesamorelin alone.

Frequently asked questions
What is tesamorelin used for?
It is FDA-approved to reduce excess abdominal fat in adults with HIV and lipodystrophy. It has also been studied for fatty liver in HIV and cognition in older adults, but those uses are not approved.
Does tesamorelin cause weight loss?
Not much. It shrinks visceral fat around the organs by about 15% over 6 months, but total weight and BMI barely change, and the label says it is not a weight-loss drug.
What happens when you stop tesamorelin?
Visceral fat comes back. In the trial extension, people switched to placebo lost most of the reduction, while people who stayed on it kept an 18% reduction at a year.
What is the difference between Egrifta SV and Egrifta WR?
Both contain tesamorelin. Egrifta SV is mixed fresh every day. Egrifta WR, approved in 2025, is a 1.28 mg dose mixed once a week, stored at room temperature, with less than half the injection volume. They are not interchangeable.
Does tesamorelin raise blood sugar?
It can. On the label, 5% of patients reached an HbA1c of 6.5% or more versus 1% on placebo, and the odds of developing diabetes were about 3 times higher. Glucose should be checked before and during treatment.
Is tesamorelin better than sermorelin?
They are both GHRH analogs, but tesamorelin is the full 44-amino-acid hormone with a protective chemical group and is approved today with phase 3 data. Sermorelin is a 29-amino-acid fragment whose approval was for children and ended in 2008. They have never been compared head to head.
