What it is

Ipamorelin is a five-amino-acid peptide that Novo Nordisk built in the 1990s. It binds the ghrelin receptor (GHS-R1a) on the pituitary, the same receptor your stomach's hunger hormone uses, and triggers a short burst of growth hormone. Its selling point in the 1998 animal work was selectivity: in pigs it released growth hormone without the cortisol and ACTH spikes seen with older peptides like GHRP-6 and GHRP-2, even at doses 200 times higher than needed.

Two human studies exist. A 1999 dosing study in 40 healthy men gave it intravenously and found a single GH pulse peaking about 40 minutes after the dose, with a half-life of about 2 hours. A 2014 phase 2 randomized trial in 114 people after bowel surgery tested whether it would get the gut moving again sooner; it did not, with no significant difference from placebo, and development for that use ended. No human study has measured muscle, fat, sleep, recovery or skin, which are the reasons it is sold online.

Ipamorelin is not approved anywhere. An FDA advisory committee voted 12-0 (one abstention) in October 2024 against allowing pharmacies to compound it, and it remains off the 503A list. FDA flags two deaths among ipamorelin-treated patients in the bowel-surgery trial, both from surgical complications, and higher rates of low potassium and high blood sugar. WADA bans it at all times.

Mechanism

How Ipamorelin works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Ghrelin receptor / growth hormone secretagogue receptor type 1a (agonist)

Ipamorelin binds the growth hormone secretagogue receptor (GHS-R1a), the ghrelin receptor, on pituitary somatotrophs and in the hypothalamus. That produces a single short GH pulse, which then raises IGF-1. In pigs it did not raise ACTH or cortisol, which separated it from GHRP-2 and GHRP-6; none of the three changed prolactin, FSH, LH or TSH in that study. Ghrelin receptors are also in the gut, which is why it was tested to restart bowel movement after surgery.

  1. 1 Injection Human studies gave it intravenously; it is sold for under-the-skin use, which has no published human pharmacokinetics.
  2. 2 Ghrelin receptor Binds GHS-R1a on pituitary somatotrophs and hypothalamic neurons.
  3. 3 Single GH pulse In healthy men, GH peaked about 0.67 hours after dosing and then fell back to negligible levels.
  4. 4 IGF-1 and tissue effects GH raises liver IGF-1. In rats, repeated dosing increased bone growth; no human tissue outcomes have been measured.
  5. 5 Gut motility In rats after abdominal surgery it shortened time to the first bowel movement; in the one human trial it did not shorten time to eating after bowel surgery.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

B Evidence

In humans, ipamorelin reliably triggers a GH pulse after an intravenous dose, and in one randomized trial it did not beat placebo for gut recovery after bowel surgery. There is no human evidence for muscle gain, fat loss, sleep, injury healing or anti-aging effects. The cortisol-sparing selectivity and bone-growth findings come from pigs and rats. The grade is B because one double-blind randomized trial exists, but that trial was negative and tested a hospital use, not the uses it is sold for.

How long it stays in the body

Half-life
About 2 hours (IV, healthy men)
Routes studied
Intravenous infusion (in studies), Subcutaneous injection (as sold, not studied in humans)
Bioavailability
Not reported in humans for subcutaneous, nasal or oral use
Notes
After IV dosing, clearance was 0.078 L/h/kg and volume of distribution 0.22 L/kg, with dose-proportional kinetics. GH peaked at 0.67 hours and returned to negligible levels; variability between people was larger for the GH response than for drug levels. In rats, 60-80% was excreted unchanged in urine.

Groups with specific risks
  • Active cancer or history of cancer: raising GH and IGF-1 is avoided with malignancy
  • Pregnancy and breastfeeding: no human data

  • Low potassium or conditions and drugs that lower potassium
  • Children and adolescents: never studied
  • Competitive athletes subject to WADA testing
Interactions
  • No formal drug interaction studies exist.
  • Combining with GHRH analogs (CJC-1295, sermorelin, tesamorelin), MK-677 or growth hormone stacks GH and IGF-1 exposure in ways that have never been tested.
  • Insulin and other glucose-lowering drugs: blood sugar rose more often on ipamorelin.
  • Diuretics and other potassium-lowering drugs: hypokalemia was more frequent on ipamorelin.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Ipamorelin releases growth hormone without raising cortisol or prolactin."MixedSome studies agree, some do not

In pigs in the 1998 study, ipamorelin did not raise ACTH or cortisol even at doses 200 times the effective dose, while GHRP-2 and GHRP-6 did. None of the three changed prolactin in pigs. Human studies did not test cortisol or prolactin, so the selectivity claim has never been confirmed in people.

The claim"Ipamorelin builds lean muscle and burns fat."UntestedNobody has measured this in people

No human study measured body composition. The claim is borrowed from what growth hormone does at much higher, sustained levels.

The claim"Ipamorelin improves deep sleep."UntestedNobody has measured this in people

No sleep study exists. Insomnia was actually more common on ipamorelin than placebo in its one trial.

The claim"Ipamorelin has no side effects."Not supportedThe evidence does not back this

In 56 treated surgery patients, low potassium, high blood sugar and insomnia were more frequent than on placebo, and FDA cited serious adverse events in its compounding review.

The claim"Ipamorelin was proven in clinical trials."Not supportedThe evidence does not back this

Its only efficacy trial, for postoperative ileus, missed its primary endpoint, and development stopped.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Ipamorelin + CJC-1295

Combines a ghrelin-receptor agonist with a GHRH analog, two separate triggers for the same pituitary cells, aiming for a bigger GH pulse.

EvidenceNever studied together in humans. FDA advisory committees rejected both for compounding.

Ipamorelin + sermorelin

Same logic as the CJC-1295 pairing, using a shorter-acting GHRH analog.

EvidenceNo clinical trials. FDA's adverse event database contains a report of a compounded ipamorelin-sermorelin product, but no efficacy or safety study exists.

Frequently asked questions
What does ipamorelin do?
It binds the ghrelin receptor in the pituitary and causes a short pulse of growth hormone. In healthy men, GH peaked about 40 minutes after an IV dose and then returned to baseline.
Is ipamorelin FDA approved?
No. It has never been approved for any use, and an FDA advisory committee voted against letting pharmacies compound it in October 2024.
Is ipamorelin legal to buy?
In the US it is sold online as a research chemical labeled not for human use. It is not an approved drug and is not on the list of substances pharmacies may compound. WADA bans it for athletes at all times.
How long does ipamorelin stay in your system?
Its half-life after an IV dose in healthy men was about 2 hours. Subcutaneous use, the way it is sold, has no published human data.
Is ipamorelin better than MK-677?
They hit the same ghrelin receptor. MK-677 is an oral non-peptide that has been through human trials; ipamorelin has no human outcome data for muscle, fat or sleep. There is no head-to-head comparison.
Does ipamorelin raise hunger like ghrelin?
It acts on the ghrelin receptor, so appetite effects are plausible, but no human study measured appetite or food intake.
