What it is

GHRP-6 is a six-amino-acid peptide that Cyril Bowers and Frank Momany designed in the early 1980s, years before anyone knew the receptor it hit. Given into a vein, it makes the pituitary release a burst of growth hormone; in an early human study, GH peaked about 45 minutes after a 30-minute infusion began and was back to baseline by about 3.5 hours. In 1996 Merck scientists cloned the receptor it acts on, and in 1999 Japanese researchers found the body's own hormone for that receptor, ghrelin. So GHRP-6 is best understood as a crude, short-acting ghrelin mimic.

What has been shown in people is narrow: single injections raise growth hormone, also raise cortisol, ACTH and prolactin at higher doses, and act together with GHRH to give a much larger GH peak than either alone. That last effect made GHRH plus GHRP-6 a diagnostic test for growth hormone deficiency in some European centers. No published human trial has tested GHRP-6 for muscle gain, fat loss, injury healing or anything else people take it for.

The healing and organ-protection claims come from Cuban and other animal studies in which GHRP-6 reduced damage to heart, liver and gut tissue, partly through a second receptor called CD36. Those results have not been repeated in human trials. GHRP-6 is banned in sport by WADA and the FDA has flagged it as a safety concern for outsourcing-facility compounding.

Mechanism

How GHRP-6 works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Ghrelin receptor GHS-R1a (agonist); CD36 (binding, animal data)

GHRP-6 binds the growth hormone secretagogue receptor (GHS-R1a), the same receptor ghrelin uses, on pituitary somatotroph cells and in the hypothalamus. It works through a different pathway from GHRH, which is why the two together release far more GH than either one alone. It also binds CD36, a scavenger receptor found in heart muscle and blood vessels, which is the proposed route for the tissue-protection effects seen in animals.

  1. 1 Binds GHS-R1a GHRP-6 attaches to the ghrelin receptor on pituitary somatotrophs and on hypothalamic neurons.
  2. 2 Amplifies GHRH, blunts somatostatin Hypothalamic action increases GHRH signaling and opposes somatostatin, the brake on GH release.
  3. 3 GH pulse The pituitary releases a GH burst; with GHRH plus GHRP-6 the peak comes within 30 minutes, and after a 30-minute GHRP-6 infusion alone GH peaked at 45 minutes.
  4. 4 Side hormones At higher doses the same receptor activation raises ACTH, cortisol and prolactin.
  5. 5 IGF-1 (with repeated use) Sustained GH release raises liver IGF-1 production; this has not been measured over months for GHRP-6 in humans.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

C Evidence

Human evidence is limited to small, mostly single-dose studies in healthy volunteers and patients being tested for GH deficiency: GHRP-6 reliably causes a GH pulse, raises cortisol and prolactin at higher doses, and stacks with GHRH for diagnosis. There are no human trials of GHRP-6 for body composition, strength, recovery or healing. The cytoprotective and antifibrotic effects are animal and cell findings only.

Human, open-label 1990

Growth hormone (GH)-releasing peptide stimulates GH release in normal men and acts synergistically with GH-releasing hormone

18 healthy men, single IV doses vs placebo

Peak GH rose from 1.2 on placebo to 68.7 micrograms/L at 1 microgram/kg; cortisol and prolactin roughly doubled at that dose, and low doses combined with GHRH acted synergistically.

Effect of a new synthetic hexapeptide to selectively stimulate growth hormone release in healthy human subjects

17 healthy men given 30-minute IV infusions of 0.05 to 2.5 micrograms/kg; 8 also received saline

GH rose consistently from 0.25 micrograms/kg, peaking at 63.0 micrograms/L at 1.0 microgram/kg; GH peaked at 45 minutes and was back to baseline by 210 minutes, with no change in ACTH, LH, FSH or TSH.

Growth hormone-releasing peptide-6 stimulates sleep, growth hormone, ACTH and cortisol release in normal man

Healthy men, placebo-controlled (randomization not reported), 4 x 50 microgram IV boluses during the night

Night-time GH rose (15.4 vs 5.5 ng/mL), ACTH rose modestly (21.0 vs 16.6 pg/mL) and cortisol more than doubled (56.0 vs 25.2 ng/mL); stage 2 sleep increased while deep slow-wave sleep did not change.

Effects of GHRP-6 on nocturnal GH, ACTH, cortisol and the sleep EEG: role of routes of administration

Healthy young men, placebo-controlled (randomization not reported); oral, intranasal and sublingual GHRP-6

Oral GHRP-6 at 300 micrograms/kg did not change GH; intranasal 30 micrograms/kg raised GH across the night, so route and dose decide whether it does anything.

A single GH determination 30 minutes after the GHRH plus GHRP-6 test is sufficient for diagnosis of somatotrope dysfunction after traumatic brain injury

83 adults at least one year after traumatic brain injury

With GHRH 1 microgram/kg plus GHRP-6 1 microgram/kg IV, the 30-minute GH value correlated with the peak at r = 0.972, so one blood draw classified 78 of 83 patients the same way.

Pharmacokinetics

How long it stays in the body

Half-life
About 2.5 hours
About 2.5 hours elimination after IV injection; the GH response is shorter, back to baseline about 3.5 hours after the start of a 30-minute IV infusion
Routes studied
Intravenous (research and diagnostic studies), Subcutaneous injection, Intranasal (research only), Oral (weak, research only)
Bioavailability
Not formally measured for subcutaneous use in humans. An oral dose of 300 micrograms/kg did not raise GH in one study, so oral absorption is very poor.
Notes
Pharmacokinetic data come from one nine-person IV study. Four of nine subjects showed unexplained concentration spikes during the elimination phase. Subcutaneous kinetics, the route most people use, have not been published.

2Commonroughly 1 in 10 or more
4Frequency unknownnot measured in trials
EffectCommonUncommonRareUnknown
Raised cortisol and ACTHSeen at 1 microgram/kg IV and with repeated night-time boluses; GHRP-6 activates the stress-hormone axis, unlike GHRH.Common
Raised prolactinProlactin roughly doubled at the higher IV dose in healthy men.Common
Hunger and increased appetiteExpected from ghrelin-receptor activation and shown with the related GHRP-2 in humans; not formally measured for GHRP-6 in published human trials.Frequency unknown
Reduced insulin sensitivity and higher blood glucoseNamed by FDA as a safety concern for GHRP-6; seen with other ghrelin-receptor agonists over weeks of use.Frequency unknown
Immune reaction to the peptide or its impuritiesFDA flags immunogenicity risk from aggregates and peptide-related impurities, a bigger issue with unregulated research products.Frequency unknown
Water retention, tingling, joint achesReported with sustained GH elevation generally; not documented in controlled GHRP-6 studies, which were too short.Frequency unknown
Groups with specific risks
  • Active cancer or a history of cancer, because sustained GH and IGF-1 elevation may promote tumor growth
  • Diabetes or prediabetes, given the risk of reduced insulin sensitivity
  • Pregnancy or breastfeeding (never studied)
  • Cushing's syndrome or other conditions with high cortisol, since GHRP-6 raises ACTH and cortisol
  • Competitive athletes subject to drug testing (WADA prohibited at all times)
Interactions
  • GHRH analogs (sermorelin, tesamorelin, CJC-1295) amplify the GH response synergistically, which is shown acutely in humans
  • Somatostatin analogs (octreotide, lanreotide) blunt GH release from secretagogues
  • Glucocorticoids and diabetes medications: GH-axis stimulation can raise glucose and change insulin needs
  • Other ghrelin-receptor agonists (GHRP-2, hexarelin, ipamorelin, MK-677) act on the same receptor; combining them adds receptor load, not a new mechanism

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"GHRP-6 builds muscle like growth hormone injections."UntestedNobody has measured this in people

It raises GH for a short window. No human study has measured muscle or strength after a course of GHRP-6, and short GH pulses are not the same exposure as daily GH injections.

The claim"GHRP-6 heals tendons, gut and organs."UntestedNobody has measured this in people

The protective effects come from animal models of heart attack, liver injury and gut damage. There are no human healing trials.

The claim"GHRP-6 makes you extremely hungry."MixedSome studies agree, some do not

Hunger is the expected effect of a ghrelin mimic and is proven for GHRP-2 in humans, but controlled human data specific to GHRP-6 appetite are thin.

The claim"GHRP-6 improves deep sleep."Not supportedThe evidence does not back this

In the one placebo-controlled sleep-lab study, GHRP-6 increased light stage 2 sleep, left slow-wave sleep unchanged, and raised night-time cortisol.

The claim"It only releases GH, nothing else."Not supportedThe evidence does not back this

At 1 microgram/kg IV it roughly doubled cortisol and prolactin in healthy men, and repeated night-time doses raised ACTH and cortisol. The 1984 animal claim of GH specificity did not fully hold in humans.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

GHRP-6 with a GHRH analog

GHRH and ghrelin-receptor agonists act on different pathways, so together they release more GH than either alone.

EvidenceThe synergy is shown for single IV doses of GHRP-6 with native GHRH in healthy men (Bowers 1990). No trial has tested this combination with CJC-1295 or measured any outcome beyond acute GH levels.

Handling

Storage and stability

Dry powder (lyophilized)

No published stability data for research-grade GHRP-6. Lyophilized peptides of this kind are generally kept sealed, dry, cold and away from light.

Mixed with water (reconstituted)

No published in-use stability data for GHRP-6 solutions. Reconstituted peptides are generally refrigerated and used within a short window; sterility depends entirely on the product and technique.

Frequently asked questions
What is the difference between GHRP-6 and GHRP-2?
Both are synthetic six-amino-acid ghrelin-receptor agonists from the same Tulane research program. GHRP-2 is more potent for GH release and is an approved diagnostic drug in Japan. GHRP-6 was the first of the family and has never been approved anywhere.
Is GHRP-6 legal?
In the US it is not an approved drug and FDA does not allow it to be compounded under the routine 503A pathway. It is sold as a research chemical not intended for human use. It is banned in sport under WADA S2.2.4.
How long does GHRP-6 last in the body?
In nine healthy men given an IV dose, the elimination half-life was about 2.5 hours. The GH pulse it triggers is shorter and peaks within about 30 minutes.
Does GHRP-6 raise IGF-1?
A single dose raises GH, not IGF-1. Repeated dosing would be expected to raise IGF-1, but there are no published multi-week human studies of GHRP-6 measuring it.
Does GHRP-6 cause hunger?
It activates the ghrelin receptor, which drives hunger, and the related GHRP-2 increased food intake by about 36 percent in healthy men. Direct controlled data for GHRP-6 in humans are limited.
Is GHRP-6 used in medicine?
Only as a research and diagnostic tool. Some European centers used GHRH plus GHRP-6 as a stimulation test for GH deficiency. It has no approved treatment use.
