What it is

GHRP-2 is a six-amino-acid synthetic peptide from Cyril Bowers' lab at Tulane, a stronger successor to GHRP-6. It switches on the ghrelin receptor, so the pituitary releases a large pulse of growth hormone within an hour. Under the name pralmorelin it is the only one of the classic GHRPs (GHRP-2, GHRP-6, hexarelin) with a regulatory approval: Japan approved GHRP Kaken 100 in October 2004 as a single 100 microgram IV injection used to diagnose growth hormone deficiency. A healthy pituitary pushes GH well above 9 ng/mL after the shot; a GH-deficient one does not.

Beyond diagnosis, the human record is a handful of small studies. Children with short stature grew faster during months of intranasal or injected GHRP-2, but IGF-1 did not rise and the trials had no placebo group. Critically ill ICU patients given a GHRP-2 plus TRH infusion for five days shifted toward protein building on blood markers. In healthy and obese adults, a GHRP-2 infusion made people eat about a third more at a buffet meal, which is the ghrelin effect showing up.

No published trial has tested GHRP-2 for muscle, fat loss or recovery in healthy adults, which is how it is used outside medicine. It also raises cortisol and ACTH. FDA says it is aware of serious adverse event reports in people given GHRP-2, including increased insulin needs, infection, pancreatitis and deaths of critically ill study subjects, though cause was not established. It is banned in sport by WADA.

Mechanism

How GHRP-2 works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Ghrelin receptor GHS-R1a (agonist)

GHRP-2 is a potent agonist of the ghrelin receptor (GHS-R1a) on pituitary somatotrophs and hypothalamic neurons. It releases GH by a route separate from GHRH, so a working pituitary responds strongly while a damaged one does not, which is the basis of the Japanese diagnostic test. Activating the same receptor also stimulates hunger and the ACTH-cortisol axis.

  1. 1 Binds GHS-R1a GHRP-2 activates the ghrelin receptor on pituitary GH-producing cells and in the hypothalamus.
  2. 2 GH pulse Serum GH peaks within 60 minutes of an IV dose; in healthy adults the average peak in the Japanese validation study was about 85 micrograms/L.
  3. 3 ACTH and cortisol The same activation raises ACTH and cortisol, which Japanese clinicians now use to screen for adrenal insufficiency during the test.
  4. 4 Appetite Hypothalamic ghrelin-receptor activation increases hunger; infusion raised buffet intake by about 34 to 36 percent.
  5. 5 Short duration Each injection gives a brief pulse; in children, injections did not raise IGF-1, so sustained effects need repeated dosing or longer-acting forms.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

B Evidence

Strong human evidence exists for one thing: a single IV dose reliably separates GH-deficient patients from healthy people, which is why Japan approved it as a diagnostic. Small randomized studies show it increases food intake in lean and obese adults, and small uncontrolled studies show faster growth in short children and better metabolic markers in ICU patients. There are no trials in healthy adults for muscle, fat loss, recovery or anti-aging.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Any effect on muscle, strength, fat mass or recovery in healthy adults; no trial has tested it
  2. 02 Whether repeated daily injections keep working or the pituitary response fades
  3. 03 Long-term effects on glucose, insulin needs, cortisol and prolactin with repeated use
  4. 04 Plasma half-life and absolute bioavailability by subcutaneous and intranasal routes
  5. 05 Whether growth gains in children seen without placebo controls are real treatment effects
  6. 06 Cancer risk from repeated GH and IGF-1 elevation
  7. 07 Quality of research-chemical products, which are not the approved Japanese product

Pharmacokinetics

How long it stays in the body

Half-life
Not established
Plasma half-life not well reported in published human studies; after an IV dose the GH peak comes within 60 minutes
Routes studied
Intravenous (approved diagnostic use in Japan), Subcutaneous injection or infusion (research), Intranasal (research), Oral (research, weak)
Bioavailability
IV for the approved product. Intranasal GHRP-2 produced GH responses above 10 micrograms/L in children at 5 to 20 micrograms/kg, but absolute bioavailability by non-IV routes has not been published.
Notes
The approved product is a single fasting IV injection. Studies in children found each injection's GH effect wore off within hours and IGF-1 did not rise, which is why the developers said a longer-acting form would be needed for treatment.

No human half-life has been published for GHRP-2, so we do not draw a clearance curve. Half-lives quoted online for it are usually estimates from animal data.

Immune reaction to the peptide or impuritiesFDA cites aggregation and peptide-related impurities, plus an unnatural amino acid that makes the molecule harder to characterize.Frequency unknown
Groups with specific risks
  • Active cancer or history of cancer, because repeated GH and IGF-1 stimulation may promote tumor growth
  • Diabetes or impaired glucose tolerance, given FDA reports of increased insulin requirements
  • History of pancreatitis
  • Pregnancy or breastfeeding (never studied)
  • Competitive athletes subject to drug testing (WADA prohibited at all times)
Interactions
  • GHRH analogs (sermorelin, tesamorelin, CJC-1295) act on a separate pathway and increase the GH response; children in one study received GHRP-2 with GHRH for 2 months
  • TRH was co-infused with GHRP-2 in the ICU studies to restore thyroid as well as GH pulses
  • Somatostatin analogs blunt GH release
  • Insulin and other diabetes medications: GH-axis stimulation can raise glucose and change dosing needs
  • Other ghrelin-receptor agonists (GHRP-6, hexarelin, ipamorelin, MK-677) act on the same receptor

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"GHRP-2 is an FDA-approved drug."Not supportedThe evidence does not back this

It is approved only in Japan, only as a single-dose diagnostic test. FDA has never approved it.

The claim"GHRP-2 builds muscle and burns fat."UntestedNobody has measured this in people

No published study in healthy adults has measured muscle or fat after a course of GHRP-2. The ICU study showed shifts in blood markers of protein turnover, not muscle gain.

The claim"GHRP-2 causes less hunger than GHRP-6."UntestedNobody has measured this in people

No head-to-head human appetite trial against GHRP-6 exists. GHRP-2 is the one with randomized human data on hunger: an infusion raised meal intake by about 34 percent.

The claim"GHRP-2 raises IGF-1 like GH injections."MixedSome studies agree, some do not

Continuous infusion in ICU patients brought IGF-1 to near-normal, but daily injections and intranasal doses in children did not raise IGF-1.

The claim"It only affects growth hormone."Not supportedThe evidence does not back this

It reliably raises ACTH and cortisol, to the point that Japanese endocrinologists use the GHRP-2 test to screen for adrenal insufficiency.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

GHRP-2 with a GHRH analog

Ghrelin-receptor agonists and GHRH act on separate pathways, so together they produce larger GH pulses.

EvidenceHuman data are limited to acute GH responses and one 2-month period in six GH-deficient children who received GHRP-2 plus GHRH. No trial has tested GHRP-2 with CJC-1295 or measured body-composition outcomes.

Frequently asked questions
Is GHRP-2 the same as pralmorelin?
Yes. Pralmorelin is the international name for GHRP-2. The approved Japanese product is GHRP Kaken 100, an injectable test vial.
What is the GHRP-2 test?
A single 100 microgram IV injection after an overnight fast, with blood drawn over the next hour or two. A peak GH of 9 ng/mL or less (recombinant GH standard) points to severe GH deficiency in Japan's criteria.
Does GHRP-2 increase appetite?
Yes. In randomized studies a 270-minute infusion made lean and obese adults eat about 34 to 36 percent more at a buffet meal.
Is GHRP-2 legal in the US?
It is not FDA-approved, FDA does not allow routine 503A compounding of it, and it is sold only as a research chemical. It is banned in sport under WADA S2.2.4.
What is the difference between GHRP-2 and ipamorelin?
Both activate the ghrelin receptor. Ipamorelin was reported in animal studies to release GH with little effect on cortisol, while GHRP-2 clearly raises ACTH and cortisol in people. Neither has trials in healthy adults for body composition.
Does GHRP-2 raise IGF-1?
It depends on how it is given. A 5-day continuous IV infusion in ICU patients brought IGF-1 near normal, but daily injections and nasal sprays in children did not raise it.
