What it is

Cerebrolysin is not a single peptide. It is a solution made by breaking down purified pig brain protein with enzymes, leaving short peptides and free amino acids; each milliliter holds 215.2 mg of that peptide fraction. EVER Neuro Pharma in Austria makes it, and it is prescribed in Austria, Russia, Eastern Europe, China and other Asian countries for stroke recovery, traumatic brain injury, and Alzheimer's and vascular dementia. It is not approved by the FDA or sold legally in the US.

It is given as a daily IV infusion in courses that start soon after a stroke or brain injury, or in repeated courses for dementia. Its makers say it acts like the brain's own growth factors, helping neurons survive and form new connections.

The human evidence is large but split. Manufacturer-supported trials and meta-analyses report better arm function and neurological scores after stroke and better recovery after brain injury. Independent Cochrane reviews found no effect on death after acute ischemic stroke, flagged a rise in non-fatal serious adverse events, and rated the dementia evidence very low quality. The biggest single trial, CASTA in 1,070 stroke patients, missed its primary endpoint.

Mechanism

How Cerebrolysin works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target Multiple: neurotrophic-factor-like signaling (proposed), no single defined receptor

Because Cerebrolysin is a mixture, there is no single receptor. The manufacturer and animal studies describe neurotrophic-factor-like effects: protecting neurons from injury, increasing synaptic plasticity, reducing infarct size and edema in ischemia models, and raising glucose transporters at the blood-brain barrier. The Austrian label notes that the peptides resemble the body's own, so blood levels cannot be measured directly.

  1. 1 IV infusion Given daily by slow IV injection or diluted infusion over about 15 to 60 minutes.
  2. 2 Peptides reach the brain Animal work suggests some components cross the blood-brain barrier, inferred from matching effects after peripheral and intraventricular dosing.
  3. 3 Neuroprotection In ischemia models it reduced infarct volume and edema and stabilized microcirculation.
  4. 4 Plasticity In injury and dementia models it increased synaptic plasticity and improved learning.
  5. 5 Clinical recovery In some human trials this translated to better motor and neurological scores, though independent reviews rate the evidence as uncertain.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

A Evidence

Cerebrolysin is an approved drug with dozens of randomized trials, which is why it earns an A for evidence volume, but the results do not all point the same way. Industry-supported meta-analyses report moderate gains in early neurological recovery after stroke and after traumatic brain injury; independent Cochrane reviews find no reduction in deaths after stroke, a possible increase in non-fatal serious adverse events, and only very low-quality evidence for vascular dementia.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Which of the many peptides in the mixture, if any, produce the clinical effect.
  2. 02 Whether the benefits seen in manufacturer-supported trials hold up in large independent trials, given CASTA's neutral result.
  3. 03 Why non-fatal serious adverse events were higher in the Cochrane stroke analysis.
  4. 04 Any benefit in healthy people; it has only been studied in stroke, brain injury, dementia and other disease states.
  5. 05 Long-term effects of repeated dementia courses over years.
  6. 06 Prion or infectious-agent risk from a pig-brain product has not been reported, but is limited by manufacturing controls rather than tested in outcome studies.

Pharmacokinetics

How long it stays in the body

Half-life
Not established
Not measurable; neurotrophic activity detectable in plasma up to about 24 hours after a dose (Austrian label)
Routes studied
Intravenous infusion, Intravenous injection (small volumes), Intramuscular injection (small volumes)
Bioavailability
100% for IV by definition; the label states that classic pharmacokinetic measurement is not possible because the peptides resemble the body's own.
Notes
Larger volumes are diluted in saline, Ringer's or 5% glucose and infused over roughly 15 to 60 minutes. Rapid injection is linked to dizziness, a feeling of heat, sweating and palpitations. It is incompatible with lipid-containing solutions and solutions that shift pH outside 5.0-8.0, and should not share an infusion with balanced amino acid solutions.

No human half-life has been published for Cerebrolysin, so we do not draw a clearance curve. Half-lives quoted online for it are usually estimates from animal data.

Non-fatal serious adverse eventsThe 2023 Cochrane review found these more often with Cerebrolysin than placebo after acute stroke (RR 2.39), and the increase was larger in the two trials that used a 10-day course.Frequency unknown
Groups with specific risks
  • Hypersensitivity to Cerebrolysin or its excipients (sodium hydroxide, water for injection).
  • Status epilepticus (Austrian label); manufacturer summaries also list epilepsy.
  • Severe kidney impairment.
  • Pregnancy, unless the clinical situation requires it (Austrian label).
  • Not recommended under age 18 because of insufficient data.
  • Caution in people with allergic tendencies or a seizure disorder.
Interactions
  • Antidepressants and MAO inhibitors: possible additive effects; the label suggests a lower antidepressant dose.
  • High-dose MAO inhibitors combined with higher Cerebrolysin doses may raise blood pressure (Austrian label).
  • Do not mix in the same infusion with balanced amino acid solutions, lipid solutions or solutions that shift pH outside 5.0-8.0.
  • Used alongside tPA (alteplase) and donepezil or rivastigmine in trials without a reported safety problem.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"Cerebrolysin is a concentrated dose of BDNF and NGF."Not supportedThe evidence does not back this

It is a digest of pig brain protein containing short peptides and amino acids. Neurotrophic-factor-like activity has been described in lab work, but it is not a vial of intact BDNF or NGF.

The claim"Cerebrolysin is proven to improve stroke recovery."MixedSome studies agree, some do not

Some trials and industry-supported meta-analyses show better early neurological and motor scores. The largest trial was neutral on its primary endpoint, and Cochrane found no effect on death and more non-fatal serious adverse events.

The claim"Cerebrolysin will make a healthy brain sharper."UntestedNobody has measured this in people

Trials enrolled people with stroke, brain injury or dementia. No controlled trial has tested it in healthy adults.

The claim"Intramuscular or subcutaneous self-injection works the same as an IV drip."UntestedNobody has measured this in people

The label allows only small volumes by intramuscular injection, while the trials used much larger daily IV infusions. Small home injections are far below any studied regimen.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Cerebrolysin with Semax

Both are used in Russian stroke care and are combined online for cognitive recovery.

EvidenceNo randomized trial has tested the two together.

Handling

Storage and stability

Dry powder (lyophilized)

Not supplied as a powder. The ready-made solution is stored below 25 C, not refrigerated or frozen, in the original carton to protect it from light. Shelf life is 48 months for 1-10 mL ampoules and 24 months for 20-50 mL vials (Austrian label).

Mixed with water (reconstituted)

Ampoules are single use. Diluted infusion solutions in saline, Ringer's or 5% glucose were tested as stable for 24 hours at room temperature; the label says to discard any unused amount immediately.

Frequently asked questions
Is Cerebrolysin FDA approved?
No. The manufacturer states it is not registered with the FDA and not approved for sale in the US. It is a prescription drug in Austria and several other countries.
What is Cerebrolysin made from?
Purified pig brain protein that has been broken down by enzymes into short peptides and amino acids, at 215.2 mg per mL.
Does Cerebrolysin work for stroke?
The results are mixed. Some randomized trials show better arm function and neurological scores, but the largest trial was neutral and the Cochrane review found no effect on death and a rise in non-fatal serious adverse events.
How is Cerebrolysin given?
By IV infusion or IV injection, once a day in courses lasting days to weeks, in a clinic or hospital. Small amounts can be given intramuscularly according to the Austrian label.
What are the side effects of Cerebrolysin?
The label lists rare dizziness, heat and sweating with fast injection, agitation or insomnia, loss of appetite, and very rare seizures and allergic reactions. It should not be used in status epilepticus or severe kidney disease.
Is Cerebrolysin banned in sport?
It is not named on the WADA list. Because WADA's S2 class covers growth factor modulators broadly, tested athletes should get a ruling from their anti-doping agency first.
