What it is

P21, usually written P021 in the papers, comes from Khalid Iqbal's lab at the New York State Institute for Basic Research. It is four amino acids (Asp-Gly-Gly-Leu) taken from the active region of ciliary neurotrophic factor (CNTF), with an acetyl group on the front and an adamantane amide on the back. The adamantane makes it fat-soluble enough to be mixed into mouse chow and still reach the brain.

In mice and rats it raised brain BDNF, increased the birth of new neurons in the hippocampus, and improved maze and object-recognition scores. In 3xTg Alzheimer's mice it lowered abnormal tau and soluble amyloid-beta. It also helped Down syndrome model mice and aged rats. Nearly all of this work comes from the one lab that invented it, and a 2024 University of Bologna study (with Iqbal as a co-author) found it did not raise BDNF or fix brain structure in living CDKL5 deficiency model mice.

No human has been given P021 in a published study and there is no registered clinical trial. Iqbal holds US patents on CNTF small-molecule peptide mimetics, as disclosed in his 2017 Down syndrome paper. What is sold online as P21 nasal spray or capsules has no human dosing, safety or effect data behind it.

Mechanism

How P21 (P021) works

What it binds to, and what happens inside the body after that, step by step.

Outside the cell Inside the cell
Target CNTF-like neurotrophic signaling, with downstream BDNF increase and GSK-3 beta inhibition (proposed)

P021 is designed to mimic the part of CNTF that supports neuron survival, without CNTF's side effects. In rodents it raises BDNF levels, which lowers the activity of the enzyme GSK-3 beta, the main kinase that adds abnormal phosphate groups to tau. The downstream effects reported are more new neurons in the dentate gyrus, more synaptic proteins, and less tau pathology.

  1. 1 Oral uptake In mice, P021 mixed into food was absorbed and reached the brain, helped by its adamantane group.
  2. 2 BDNF rises Treated rodents showed higher brain-derived neurotrophic factor in the hippocampus.
  3. 3 GSK-3 beta falls BDNF signaling reduced the activity of GSK-3 beta, the enzyme that hyperphosphorylates tau.
  4. 4 Neurogenesis and synapses More new neurons matured in the dentate gyrus and synaptic proteins increased.
  5. 5 Behavior in rodents Mice and rats improved on maze and novel-object tests; no human data exist.
The research

What the studies show

Human results and animal results are kept apart below. A result in rats tells you a mechanism is plausible. It does not tell you what happens in a person.

D Evidence

All evidence is from mice, rats and cell lines, and almost all of it comes from the lab that created P021. The rodent results are consistent across Alzheimer's, aging and Down syndrome models, but a 2024 study led by a different group, in a different disease model, saw little benefit in living mice. There is no human data on safety, dose or effect.

Gaps in the research

What nobody has measured yet

These are questions the published research has not answered.

  1. 01 Any effect at all on memory or cognition in humans.
  2. 02 Safe dose, half-life and brain exposure in people, by any route.
  3. 03 Whether the BDNF increase seen in mice happens in humans, and whether a sustained BDNF or neurogenesis push has downsides.
  4. 04 Whether results hold up outside the originating lab, given that the one study led by another group saw little benefit in living mice.
  5. 05 Whether online 'P21' products are actually the adamantane-capped P021 molecule used in the studies.

Pharmacokinetics

How long it stays in the body

Half-life
Unknown in humans
Routes studied
Oral, mixed into food (rodent studies), Peripheral injection (early mouse studies)
Bioavailability
Reported to cross the blood-brain barrier and work orally in rodents; never measured in humans.
Notes
Rodent studies dosed P021 continuously in chow for months. No human pharmacokinetic study exists, and the intranasal route sold online has not been tested in published research.

No human half-life has been published for P21 (P021), so we do not draw a clearance curve. Half-lives quoted online for it are usually estimates from animal data.

Dosing data

Doses used in published studies

No human study or approved label has published a dose for P21 (P021), so there is no table to show.

Studied doses

No dose of P21 (P021) has been tested in a published human study or approved on a label. Numbers that circulate online come from animal studies scaled up by guesswork or from sellers. We do not reproduce them.

Safety

Side effects, higher-risk groups and interactions

EffectCommonUncommonRareUnknown
Human side effectsNever given to humans in a published study.Frequency unknown
Toxicity in rodentsThe originating lab reports no obvious toxicity in long-term mouse feeding, and one study saw lower mortality in treated 3xTg mice, but no formal toxicology package has been published.Frequency unknown
Groups with specific risks
  • No established contraindications because there is no human safety data.
  • Pregnancy: P021 was given prenatally in Down syndrome model mice, but this says nothing about safety in human pregnancy.

Claims checked

What people say online, checked against the studies

Tap a claim to see what the studies found.

The claim"P21 grows new brain cells in humans."UntestedNobody has measured this in people

Increased neurogenesis was measured in mouse and rat hippocampus tissue. No human study has looked.

The claim"P21 is a proven Alzheimer's treatment."Not supportedThe evidence does not back this

It reduced tau and amyloid in transgenic mice. Many compounds have done that and failed in people; P021 has not been tested in a single patient.

The claim"P21 nasal spray is the studied form."Not supportedThe evidence does not back this

The published studies gave P021 in food or by injection to rodents. No paper tests an intranasal product.

The claim"P21 raises BDNF."MixedSome studies agree, some do not

Several rodent studies from the originating lab found higher BDNF, but a 2024 Bologna-led study in Cdkl5 knockout mice did not. No human measurements exist.

Untested pairings

Combinations people discuss

These pairings come up often. Almost none have been tested together in a trial, and the evidence note on each says so plainly.

Handling

Storage and stability

Dry powder (lyophilized)

No published stability data exist for the research-grade powder. Research suppliers generally ship it as a freeze-dried powder and list frozen storage (-20 C), dry and away from light; none of this has been independently verified.

Mixed with water (reconstituted)

No published stability data exist for a reconstituted solution. The usual lab practice for small peptides is refrigeration at 2-8 C and short use windows, but no study has tested how long this compound stays intact once mixed.

Frequently asked questions
Is P21 the same as P021?
In the peptide market, yes: both names refer to the adamantane-capped CNTF fragment from the Iqbal lab. The 2010 paper called it P21; later papers call it P021. It has nothing to do with the p21 cell-cycle protein.
Has P21 been tested in humans?
No. There is no published human study and no registered clinical trial. Everything known comes from mice, rats and cell culture.
How is P21 different from Cerebrolysin?
Cerebrolysin is a mixture of pig-brain peptides given by IV infusion and approved as a medicine in several countries. P021 was designed to mimic one region of CNTF and is a single synthetic molecule with no approval anywhere.
Is P21 legal?
It is not approved as a drug anywhere and is not on any US compounding list. It is sold as a research chemical. For tested athletes it falls under WADA's S0 ban on unapproved substances.
Does P21 work for anxiety or depression?
There is no study of P21 for mood in animals or humans. The research has focused on memory, Alzheimer's pathology, aging and Down syndrome models.
